Competing interests CAH and EM are employees of Enteromed Ltd, the sponsor of the study. PP, VA, AK and RA have no conflicts of interest to declare. AVK is an employee of TNK SILMA LLC, manufacturer of Enterosgel in Russia. Purpose of the study: To see if Enterosgel helps to treat the main symptoms of people with irritable bowel syndrome with diarrhoea (IBS-D). Enterosgel is free from preservatives, gluten, sugar, sweeteners, lactose, fat, flavours, colouring, plant or animal-derived ingredients. Mix a dose of Enterosgel (a teaspoon or a table spoon depending on the age and indication) in a glass of water and take 2 hours before or after a meal or taking medications. People taking part: 440 people aged 16 to 75 years, diagnosed with irritable bowel syndrome with diarrhoea (IBS-D) took part in this UK trial.

Double-blinded randomised placebo controlled trial of - Gut

Enterosgel just contains an organic mineral and purified water. It does not contain anything else so is free from drugs, preservatives, colouring, gluten, lactose, flavours or sweeteners, and does not have a taste. What was the primary endpoint: The primary endpoint is the main result that is measured to see if the treatment has worked. Overall, the results show that Enterosgel ® is an effective treatment and can improve quality of life for IBS-D patients, many of whom experience incapacitating symptoms. Our primary outcome analysis used multiple imputation for missing diary entries for both stool consistency and abdominal pain. Although this method has been widely adopted to handle missing data 35 it can introduce bias, but it is reassuring that the resulting ORs were comparable to the observed data.

This study had limitations namely that the median duration of diarrhoea prior to randomisation was 86 hours (min–max: 7–690) as there was no upper limit in the exclusion criteria. The Department of Health guidance for adults with acute diarrhoea is to only visit their GP if they have diarrhoea for more than 7 days. The delay could have impacted on the measured efficacy of the treatment. The secondary outcome diarrhoea duration until the last liquid stool was only available for those patients who had a liquid stool after randomisation and resulted in an underpowered sample size, which may have contributed to the non-significance. These may have been avoided by capturing patients at an earlier stage; for example, from pharmacies, before patients made an appointment with their GP. This study was not placebo-controlled, although it was compared with current recommended standard of care, which can be justified for studies wanting to provide evidence of improved efficacy of a new therapy over an existing therapy. 22 Moreover, outcomes were standardised to minimise any bias and based on patient-reported outcomes.

IBS – Enterosgel can normalise stool frequency and provide relief from constipation, one of the issues associated with irritable bowel symptom. The Microflora is paramount when it comes to gut health and Enterosgel helps to regulate and protect it. In children, treatment of diarrhoea must be accompanied by the early administration of the oral rehydration solution (ORS) to avoid dehydration. Since eligibility was assessed according to the current Rome IV criteria, the applicability of our findings to the estimated 50% of patients with IBS not matching the Rome IV criteria, 34 including those with milder pain, is not known. We may, however, have mitigated for these patients by recruiting a proportion of cases directly from the community setting and when we reduced the eligibility abdominal pain score at screening to lower screening failures. Following 8 weeks of open-label treatment, 349 patients completed the open-label phase. Comparison with secondary outcome data reported during the double-blind phase confirmed continued treatment effect for the same observed variables ( online supplemental files S7–S10). Mean days per week loperamide use fell in the Enterosgel group in contrast to the placebo (week 1: 0.83 vs 1.14, week 8: 0.77 vs 1.47) and continued to fall during open label (week 16: 0.68 vs 0.87), suggesting patients experienced a continued improvement in symptoms. No, Enterosgel is classified as a medical device and has a CE certificate for use in Europe. It is a medical device and not a medicine as Enterosgel is not absorbed into the bloodstream from the gut but travels along the inside of the gut before it is completely excreted with the stools. Enterosgel does not cause any systemic side effects. Enterosgel contains unique, porous molecules, which pass gently through the gut without being absorbed themselves. The pore size of these molecules is designed to selectively attract and bind harmful particles on its surface, without removing beneficial substances such as water or vitamins.
