The gameplay originally focuses on the Office of Strategic Services (OSS), an American espionage organization during World War II, placing emphasis on using false papers and silenced pistols, but as the series and the technology has progressed, it has shifted emphasis towards front-line combat. Since the release of Medal of Honor: Rising Sun the series has focused on providing a more open-ended element to the games, allowing the player to have more options in each level instead of a linear path. The violence, up to Medal of Honor: Rising Sun is bloodless and simple, usually consisting of elaborate animations, while the violence in Medal of Honor: Pacific Assault and onward contains the occasional sprays of blood. To make the game look and feel authentic and historically accurate, the developers consulted the retired U.S. Marine Captain Dale Dye, a veteran of the Vietnam War. Dye put the developers through the same military training he put the creators of Saving Private Ryan through. Actual World War II footage was used during the in-game briefings and cutscenes to immerse the player in the time period. The developers even went so far as to record the actual sound the weapons made when shot. Footage of the training can be watched in the special features of Medal of Honor: Frontline, entitled, A Good Day to Dye. In addition to featuring a single-player campaign mode and an arcade-style rail shooter mode, both versions of the game have extensive multiplayer modes. Both versions can support up to 32 players online on a single server, with six maps and six uniforms, three for Axis and three for Allies. The uniforms for the Allies are "Ranger's 2nd", "Ranger's 4th", and "Ranger's 5th", whereas the Axis uniforms are simply "Axis Rookie", "Axis Regular", and "Axis Elite". There are three different online multiplayer modes available: [1] free-for-all deathmatch, team deathmatch and Infiltration. While the Wii version's multiplayer is online-only, with no offline split-screen multiplayer modes, the PSP version supports local wireless multiplayer among up to 8 players.

Take the Airfield - Take a small squad and shut down the German airfield by first destroying their radio tower to cut off communications and then disabling the base generator. Skirmish is a game mode unique to Medal of Honor: Heroes. It is an offline mode where the player can battle to up to 16 other AI enemies. The maps are the same areas as the singleplayer missions and the player can choose their uniform and primary weapon (pistols and grenades vary by faction) before and during a match.
